Angel Health and Wellness Clinic is committed to providing quality care by offering the highest level of service.

For new appointments, please complete paperwork prior to your arrival as well as arrive to your appointment 10 minutes early.

A late arrival may need to be rescheduled or may decrease the time necessary for your treatment and may potentially limit the effectiveness of the treatment.

We do not bill insurance, however, an itemized bill can be supplied to you to be submitted to your insurance for possible reimbursement (no guarantee of reimbursement - depends on your insurance carrier). Credit card, cash, Paypal, Venmo, and HSA (Health Savings Account) are accepted.

Cancellation 

As a courtesy to other patients and to your provider, please provide adequate notice (48 business hours for new patient appointments and procedures and 24 business hours for follow up appointments) if you need to cancel or change an appointment. 

• Cancellations without adequate notice or no-shows will be subject to a missed appointment fee of  $150.00 for new patient appointments and $100.00 for follow-up appointments and/or forfeiture of the prepaid service.
