Cancellation Policy

As a courtesy to other patients and to your provider, please provide adequate notice (48 business hours for new patient appointments and procedures and 24 business hours for follow up appointments) if you need to cancel or change an appointment. Cancellations without adequate notice or no-show will be subject to a missed appointment fee of $150.00 for new patient appointments and $100.00 for follow-up appointments and/or forfeiture of the prepaid service.
